This podcast series is an educational program designed to support physicians and advanced practice providers by addressing real‑world challenges related to clinical documentation and coding. Episodes focus on high‑impact topics that directly affect clinical practice, including documenting medical decision‑making, capturing patient complexity, avoiding common documentation pitfalls, and aligning documentation with current Medicare and payer expectations.

Target Audience

This course is intended for physicians, physician assistants, and all other advanced practice clinicians regardless of specialty or sub-specialty

Learning Objectives

At the end of this session, learners should be able to:

  1. Differentiate between documentation that reflects clinical activity versus documentation that clearly supports medical decision‑making or time for E/M services.
  2. Apply current E/M documentation principles to real‑world clinical scenarios to accurately reflect patient complexity and decision‑making.
  3. Select the appropriate method for E/M coding (medical decision‑making or time) based on the work performed and document it clearly.
  4. Demonstrate how to explicitly document clinical reasoning, risk assessment, and management decisions in the medical record.
  5. Identify and correct common documentation gaps that lead to E/M audit risk or inconsistent code support.
  6. Implement documentation practices that consistently align with current Medicare and payer expectations without increasing documentation burden.
