The Pharmacy Patient Safety Series provides our pharmacy teammates with continuing education. Various topics will be presented to assist with advancing best practices for medication-related patient safety. Concepts taught may include high reliability principles, effectiveness frameworks, safety culture, and risk mitigation strategies, as well as other medication safety topics.

Overall Objectives:

1. Identify organizational strategies to reduce patient harm from medication use. 
2. Describe key concepts related to medication safety, such as high reliability, error science, and just culture.
3. Outline existing best practice resources for medication safety, including those provided by the Institute for Safe Medication Practices (ISMP). 
4. Discuss applicable medication safety policies to ensure medication safety. 

Target Audience:  
This activity is designed for pharmacists, pharmacy technicians, and other healthcare professionals.
