Description: This presentation will review key medication safety principles and processes that support a culture of safety within healthcare organizations. Participants will learn the importance of reporting medication events, understand the purpose of root cause analyses (RCAs), recognize the signs and symptoms experienced by second victims, and explore ISMP Targeted Medication Safety Best Practices for Hospitals and Community Pharmacy. The session will also provide an overview of relevant medication safety policies and their role in preventing medication errors and improving patient outcomes.
