Description: This session provides an overview of medication safety through the lens of high reliability principles, Just Culture, and applied error science. Participants will explore how human error, system design, and event reporting contribute to patient safety, and learn how to differentiate error types and respond using the Just Culture framework. The program emphasizes practical strategies such as the use of the Institute for Safe Medication Practices (ISMP) Hierarchy of Effectiveness to design safer systems and reduce the risk of harm.

  2. Explain how Just Culture and the Just Culture Decision Guide distinguish between human error from unsafe behavior
  3. Describe how reporting safety events supports system improvement rather than individual blame
  4. Classify proposed safety solutions using the Hierarchy of Effectiveness Framework
