This presentation explores the impact of social determinants of health (SDOH) on patient outcomes, particularly within the context of pharmacy practice. Attendees will gain an understanding of SDOH, their connection to health disparities, and actionable strategies for pharmacists to intervene and promote health equity. The session will also highlight resources and the organization's role in supporting both healthcare providers and patients in addressing SDOH.

At the end of this session, learners should be able to:

  1. Define social determinants of health (SDOH) and their relationship to health disparities.
  2. Identify opportunities for pharmacist interventions and provide recommendations to address SDOH.
  3. Utilize available resources to support teammates and patients in achieving health equity.
  4. Outline the organization’s role in promoting health equity and addressing SDOH.
