Overview
Respecting Choices Next Steps Advance Care Planning (ACP) is a training offered to healthcare workers who provide care to patients with chronic conditions. Next Steps Advance Care Planning is an activity aimed at assisting patients in exploring their hopes, values and beliefs about their disease progression and possible treatment options. This course will provide training to healthcare workers to prepare and train them to facilitate these care planning discussions.

Target Audience
This course is intended for an interprofessional audience that includes Nurses, Social Workers, Clergy/Faith Leaders, and others involved in Advance Care Planning. Target audience is professionals seeking certification in Advance Care Planning.
Learning Objectives
At the end of this session, learners should be able to:
Identify ways to have person centered conversations with patients and families on ACP goals, values and preferences, be able to apply it into a written plan and into our documentation system.
