Overview

The norm of monitoring anticoagulant therapy may not be appropriate from patient to patient based on disease state with discrepancies in particular labs being drawn. This presentation will provide an overview in to commonly used anticoagulant therapy and alternative monitoring strategies that can be utilized for more specialized populations.

Learning Objectives

At the end of this session, learners should be able to:

  1. Outline the mechanism of action of commonly used anticoagulant therapy, including unfractionated heparin, low molecular weight heparin, and direct oral anticoagulants.
  2. Evaluate how commonly used anticoagulants can be monitored in current practice.
  3. Distinguish special populations where current anticoagulant monitoring and dosing may be inaccurate.
  4. Develop a monitoring strategy for anticoagulants in special populations.
