High Reliability Leadership Series: Make-up Session Seven

Medical errors are third leading cause of death in the U.S. and 10 percent of U.S. deaths are due to preventable medical mistakes. Advocate Aurora Health has declared patient safety a moral imperative for our patients. As such, we have committed to a goal of ZERO serious safety events. In these sessions we will work on eliminating serious harm throughout our healthcare delivery system by implementing a culture of high reliability performance. 

Target Audience

This course is intended for all medical specialties.

Learning Objectives

At the end of this session, learners should be able to:

  1. Develop an understanding of how shared, accurate, and precise knowledge of the dynamics and location of the current operating point and boundary locations is a necessary component of a safety culture.
  2. Distinguish between first- and second order problem-solving behavior
  3. Review the leader’s role in organizational learning, including the AAH Cause Analysis process
  4. Build awareness on failure mode statements to drive systems thinking
