Health Equity - APP
This course aims to assist care providers in laying the foundation to advance Health Equity (HE). We focus on one aspect of health disparity, hypertension, and define steps providers can take to reduce inequities among our minority populations.
Target Audience
This course is intended for APP physicians.
Learning Objectives
At the end of this session, learners should be able to:
- Define Health Equity
- Cite what AAH is doing to promote it across the system
- Describe the Hypertension pilot study 2021
- Define Social Determinants of Health or SDOH barriers
- List steps providers can take to help patients overcome SDOH barriers
- Define Health Literacy
- Describe Red Flags for health literacy
- Describe what it means to be health literate organization
- List steps providers can take to help patients overcome limited health literacy
