The Gut Stops Here: Getting things moving again

The purpose of this program will be to educate and inform system pharmacists about the gastrointestinal complications that arise in our critically ill and post-surgical patient populations. The topics that will be covered will include a brief introduction on feeding in these complex patient populations, as well as the pathophysiology and pharmacologic management of constipation, post-operative ileus and Ogilvie's Syndrome. In addition, medications used to treat these conditions and patient specific monitoring that is required with drugs such as neostigmine, alvimopan and methylnaltrexone will be discussed. The activity will describe the purpose of the REMS program associated with alvimopan, and lastly go into our ERAS/STAAR post-operative enhanced recovery protocols that are necessary to alleviate and prevent the aforementioned complications.

Learning Objectives

At the completion of this activity, the participant will be able to

  • Evaluate the role of nutrition in the prevention of gastrointestinal complications in critically ill and surgical patient populations.
  • Compare and contrast the pharmacologic management of Ogilvie’s Syndrome, ileus, and constipation.
  • Analyze the usage patterns, monitoring criteria, dosing strategies, and place in therapy for alvimopan, neostigmine, methylnatrexone.
  • Apply the Enhanced Recover After Surgery (ERAS) and Surgical Team Approach to Advanced Recovery (STAAR) protocol to a patient case scenario.
