Conversations in Serious Illness: Aligning Care with Patient Goals

This course is designed to help clinicians initiate and navigate person-centered conversations in serious illness using an evidence-based framework and patient-tested language. Through a blend of foundational knowledge, structured guides, and practice scenarios, they will learn to communicate prognosis compassionately and align care with individual values and preferences.

Target Audience

This course is intended for physicians, nurses, social workers, physician assistants, and other healthcare professionals caring for seriously ill patients.

Learning Objectives

At the end of this session, learners should be able to:

  1. Describe the evidence-based benefits of having serious illness conversations for patients, families, substitute decision makers and providers.
  2. Identify common barriers to effective serious illness conversations.
  3. Define the components of the Ariadne Labs's Serious Illness Conversation Guide.
  4. Describe how to deliver a prognosis to a seriously ill person.
  5. Use the Serious Illness Conversation Guide in practice exercises.
