AboutHealth: Post-Acute Care Collaborative
In this program you will learn how to identify best practices in the specific areas of establishing and/or enhancing discharge disposition protocols, as well as relationships with outside facilities (such as Skilled Nursing Facilities (SNF), Home Health Agencies, etc.) to influence and/or manage the care of our patients across the continuum.
Learning Objectives
At the end of this session, learners should be able to:
- Agree on a set of action items that will be implemented to improve performance in readmissions and ensure patients discharged to the most appropriate care setting.
- Demonstrate Clinical Integration by agreeing on and implementing a set of best practices at all member sites.
