AAH Nursing Grand Rounds Recordings 2025: Innovative Approaches to Health Care at "Home"
Advocate Health is a leader in designing new nurse-led models of health care – developing and maintaining relationships using in-person and virtual visits to help patients and clinicians manage chronic disease outside of traditional settings. These presenters will provide an overview of several innovative strategies including “Hospital at Home” and the Comprehensive Annual Visit (CAV) program and other strategies used across the Advocate Health footprint.
