AAH Nursing Grand Rounds Recordings 2025: Breaking Down Barriers to Medication Adherence
Over 50% of Americans have trouble taking their medications as prescribed, resulting in poor health outcomes and significant cost for patients and the health care system. This program will feature nurses who use telephonic care coordination to educate, influence, and help people to manage their medications more effectively.

- Define medication adherence and describe how it is measured and reported
- Describe how to use a screening tool and motivational interviewing to enhance adherence
- Discuss strategies nurses can use to ensure follow-up after home discharge.
