As cancer treatment continues to shift beyond traditional clinical settings, healthcare professionals are increasingly challenged to provide high-quality, evidence-based care in patients' homes. The 2nd Annual Elevating Care from Clinic to Couch: Bringing Cancer Care Home Conference is designed to equip clinicians with practical strategies, current evidence, and interdisciplinary approaches to optimize outcomes for patients living with cancer across the continuum of care. This engaging educational event brings together experts in oncology nursing, pharmacy, nutrition, rehabilitation, and supportive care to address the unique clinical, psychosocial, and functional challenges faced by patients and their caregivers in the home setting. Participants will explore the impact of caregiver burden and financial toxicity, gain insight into the management of cancer-related cognitive impairment, learn nutrition strategies to mitigate treatment side effects, and discover rehabilitation interventions that improve function and reduce cancer-related fatigue. Through expert presentations, interactive question-and-answer sessions, and interdisciplinary discussion, attendees will leave with actionable tools to enhance patient-centered care, support caregivers, improve symptom management, and promote quality of life for individuals receiving cancer care at home.

Target Audience
This course is intended for Registered Nurses, Advanced Practice Providers, Physicians, Rehabilitation Professionals, Home Health Clinicians, Hospice Professionals, and other healthcare providers involved in the care of patients with cancer.
Learning Objectives
At the end of this session, learners should be able to:
- Identify psychosocial, emotional, and financial challenges experienced by patients with cancer and their caregivers in the home setting
- Apply evidence-based strategies to recognize and manage cancer-related cognitive impairment ("chemo brain").
- Implement nutrition interventions to address treatment-related side effects and support optimal nutritional status.
- Integrate rehabilitation principles and exercise-based interventions to reduce cancer-related fatigue and improve functional outcomes.
- Collaborate across disciplines to deliver safe, effective, and patient-centered cancer care in the home environment.
