All of these are required (and need to be accurate) to submit continuing pharmacy education to CPE monitor.

Physicians: Some programs available on this platform are certified to provide Maintenance of Certification (MOC) credit. To report MOC credit to your certifying Board, your profile must include your diplomate information including your Board(s), your unique Board ID number(s), and your date of birth. You may enter multiple Board(s). By completing the information below, you give permission for your diplomate information to be shared with the Accreditation Council for Continuing Medical Education (ACCME) for the purpose of reporting educational credit to your certifying Board(s). This information (your certifying Board(s) and DOB) is being collected solely for the purpose of reporting completions to your certifying Board(s).
