SUBMIT A QI PROJECT POSTER

Requirements

1. Poster must include an adequate and appropriate Aim Statement/SMART Goal (Specific, Measurable, Achievable, Relevant, and Time-bound).
2. Poster must include adequate and appropriate measures that are aligned to determine if change occurs and relate directly to AIM Statement/SMART Goal. 
3. Poster must include adequate and appropriate interventions that have been aligned to impact the measures and were implemented into practice.
4. The poster cannot reflect work that was already submitted for MOC Part IV credit. 
5. The poster must have been presented, shared, or published at a quality week, conference, small group session, or other organizational event. 
6. Attestations are required for both listed poster authors and those who participated in the improvement work but are not listed as authors. 
7. The physician or PA seeking credit for a QI project poster must adhere to all criteria of meaningful participation.

Process

1. Ensure that your poster meets the above requirements.
2. Complete the QI Poster Attestation by November 14th to receive credit for this calendar year.
3. Advocate Health Interprofessional Continuing Education (IPCE) team will review and submit the attestations to ABMS and the certifying board(s).
