Social Workers Driving Outcomes in Health (SDOH): The integral role of social work practice in addressing the social drivers of health - Social Work Month Event
Social Workers Driving Outcomes in Health (SDOH) is designed to deepen the expertise of social work on the Social Determinants of Health (SDOH) and their impact on health outcomes. This program brings leading voices and practical tools for addressing SDOH challenges in healthcare settings, equipping attendees to drive meaningful change and enhance overall patient well-being. Attendees will gain insights into evidence-based strategies for SDOH integration, lean best practices for promoting health equity, and foster connections to strengthen collaboration withing the healthcare community.
This conference is being held both in-person and virtually.

Target Audience
This course is intended for social workers.
Learning Objectives
At the end of this session, learners should be able to:
- Identify key social drivers of health and their impact on patient well-being.
- Implement evidence-based strategies to address social determinants of health in various practice settings.
- Collaborate effectively with healthcare team members to advocate for SDOH-informed patient care.
- Evaluate the effectiveness of social work interventions in improving patient outcomes related to SDOH.
- Develop action plans for integrating SDOH considerations into patient assessments and care plans.
