Geriatrics for Advanced Practice 2021 Webinar Series - Geriatric Care for the Community and Beyond: Functional Assessment and Transitions Planning for the Hospital Patient

Geriatrics for Advanced Practice is a unique educational activity that is intended to enhance the knowledge and ability of advanced practice providers to be able to contribute to the health of a population and provide optimal evidence-based care for the high-risk population of older adults. 

Target Audience

This course is intended for Advanced Practice Clinicians NPs, PAs, Physicians, Nursing, and other interested healthcare professionals.

Learning Objectives

At the end of this session, learners should be able to:

  1. Describe functional needs during the acute care transitions planning.
  2. Integrate what ‘matters most’ as they develop safe transitions planning.
  3. Integrate medication management strategies during safe transitions planning.
