1. Login to the CE Learning Platform.

• Advocate Health Teammates
  • If you have an Advocate Health teammate, click Advocate Health Login and use the log in link applicable to your login credentials
• All Other Users
  • If you are NOT an Advocate Health teammate, click Visitor Login
  • ​​​​​Fill in the required fields, then click Log In.

2. On the home page hover over the Resource tab. Under forms click on Disclosure.

3. Complete the form and click on Submit.
