The Care Transitions Webinar Series is designed to equip nurses, social workers, and other healthcare professionals with the latest updates and practical guidance for managing patient care transitions. This series will provide comprehensive clarification on best practices, ensuring attendees are well-informed about current standards and protocols in both nursing and social work.

Association of Social Work Boards (ASWB)
As a Jointly Accredited Organization, Advocate Health is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 3.75 ethics and general continuing education credits.
