Overall Objectives:

  1. Identify complex heart failure presentations for early signs in order to guide timely, evidence-based interventions.
  2. Integrate multidisciplinary approaches into clinical practice for patients with high acuity and multiple comorbidities.
  3. Apply case-based strategies to improve diagnostic accuracy and optimize treatment plans for advanced heart failure patients.
