Program Overview:

A series of facilitated virtual presentations that includes care management patient cases, care manager’s use of MI skills, and question-and-answer opportunity.

  1. Formulate at least 1 way to apply MI skills within patient and/or team member encounters such as difficult conversations, life-style changes, and patient engagement.

Target Audience:  This activity is designed for Ambulatory Care Management nurses and social workers and other interested Ambulatory Care Management team members

Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 1.0 Clinical continuing education credits.
