Program Overview:
A series of facilitated virtual presentations that includes care management patient cases, care manager’s use of MI skills, and question-and-answer opportunity.

Overall Objectives:
- Formulate at least 1 way to apply MI skills within patient and/or team member encounters such as difficult conversations, life-style changes, and patient engagement.
Target Audience: This activity is designed for Ambulatory Care Management nurses and social workers and other interested Ambulatory Care Management team members
