Description:

Falls are a leading cause of injury, loss of independence, and healthcare utilization among older adults. The Community Falls Prevention ECHO series addresses this critical public health issue by strengthening access to evidence-based fall prevention education for professionals who serve older adults. The series is being led by Aurora Health Care and is supported by a 5-year Geriatric Workforce Enhancement Program (GWEP) grant.  This series will be developed in partnership with regional fall prevention coalitions and community partners, with the shared goal of expanding access to high‑quality fall prevention education

Overall Objectives:

1.  Identify evidence-based strategies to assess fall risk and implement fall prevention interventions for older adults across healthcare and community settings.  
2.  Describe practice improvements that strengthen screening, referral, and care coordination processes for older adults at risk of falls. 
3.  Apply fall prevention best practices and community resources to real-world cases through interdisciplinary discussion and case-based learning

This activity is designed for physicians, nurses, social workers, and other healthcare professionals.
