Overall Objectives:
- Apply most recent and up to date, evidence based knowledge in the management of patients
- Apply current practice guidelines in managing patients with end stage renal disease with different modalities of dialysis, home hemodialysis, peritoneal dialysis and in center hemodialysis
- Improve patient outcomes and identify risk factors for readmission and avoid near miss cases in the management of patient with cardiorenal syndrome and diuretic management as outpatient
