Overall Objectives:

At the end of this session, learners should be able to:

  • Identify aligned clinicians and practices with the greatest opportunities for improvement in Quality performance metrics
  • Develop strategies and tactics to improve engagement and alignment with APP’s goals and objectives in value-based care initiatives
  • Understand barriers unique to independent practices which may be impacting performance
  • Provide appropriate support, resources and mentorship for performance improvement across all PHOs

Target Audience:  
This activity is designed for the physician and nurse members of the APP QI Committee.
