Overview: Nearly half of adults in the United States have uncontrolled high blood pressure, which is a leading cause of heart disease, stroke and kidney disease. Nationally, only 1 in 4 adults have their blood pressure well-controlled and among Black U.S. adults that drops to 1 in 5 adults. Our organization has prioritized reducing disparities in uncontrolled hypertension (HTN) with a goal to close the gap among our communities of color by 20% in 2023. During this webinar, a high-level overview of our system dashboard, measures, tools and steps will be shared for you to promote equitable hypertension control in your clinical practice.

At the end of this session, learners should be able to:

• Demonstrate an understanding of the system’s Health Equity Uncontrolled Hypertension measure on the system DE&I Dashboard.
• Learn about the Health Equity Uncontrolled Hypertension SMART Action Plan developed to help close the gap in uncontrolled high blood pressure among communities of color.
• Identify tools and steps physicians and APCs can take to promote equitable hypertension control in clinical practice, including MyChart Vitals tracking for home self-measured blood pressure monitoring.
