Course Overview: This presentation reviews evidence and discusses management strategies for myocardial revascularization in patients with severe ischemic cardiomyopathy, comparing CABG, PCI, and guideline‑directed medical therapy.

- Evaluate evidence for revascularization strategies in ischemic cardiomyopathy, including CABG, PCI, and guideline‑directed medical therapy, with emphasis on survival and quality‑of‑life outcomes from major trials and registries.
- Assess the clinical role of myocardial viability and ischemia testing in patients with severe LV dysfunction, understanding their strengths, limitations, and lack of consistent interaction with long‑term mortality benefit.
- Apply a patient‑centered, heart‑team–based revascularization algorithm to individualize treatment decisions, balancing anatomic complexity, surgical risk, symptoms, and patient preferences.
