INITIATE A NEW PROJECT

Requirements

  1. The physician or PA seeking to initiate a QI project must be affiliated with Advocate Health. Only one project approval needs to be submitted regardless of the number of physicians or PAs seeking credit. 
  2. The QI project submitted must reflect the priorities of Advocate Health or address facets of health care continuum, including but not limited to: 
  • Processes (i.e.: transition of care, discharge planning, advanced directives)
  • Clinical Care (i.e.: sepsis, depression, opioid treatment)
  • Administrative/Systems (i.e.: patient communication, EMR use, resource allocation)
  • Population Health (i.e.: diabetes, food safety, health disparities)
  1. The QI project must utilize an improvement methodology (i.e.: PDSA, IHI Collaborative Model, CQI).
  2. The QI project must include an oversight process that ensures meaningful participation.
  3. The physician or PA seeking credit must adhere to all criteria of meaningful participation.

Process

  1. The QI project lead for a quality improvement project must submit the QI Project Application by October 17th to receive credit in the same calendar year. This can be done before the project starts, during the project, or after the project is complete. 
  2. Advocate Health Interprofessional Continuing Education (IPCE) team will review the application and may request additional information. Once the application review is complete, the IPCE team will submit the project to ABMS and the certifying board(s).
  3. ABMS and certifying board(s) will review the project application. The IPCE team will notify the physician or PA who submitted the application once approved. 
  4. Each physician or PA seeking credit must complete the QI Project Completion Attestation by November 14th
  5. IPCE team will review and submit the attestations to the certifying board(s).

 

JOIN AN ACTIVE PROJECT

Requirements 

  1. The physician or PA seeking credit must be affiliated with Advocate Health.
  2. The QI Project must be pre-approved by the board the physician or PA is seeking credit from. 
  3. The physician or PA seeking credit with an already approved QI project must adhere to all criteria of meaningful participation.

Process

  1. The interested physician or PA will review and select one of the approved efforts relevant to their practice.
  2. The physician or PA should contact the QI project lead for further information. 
  3. Each physician or PA seeking credit must complete the QI Project Completion Attestation by November 14th.
  4. Advocate Health Interprofessional Continuing Education (IPCE) team will review and submit the attestation to the certifying board(s).
