Topic:  Person-Centered Goals of Care Conversations

Course Overview:   This session provides practical, evidence‑based strategies to lead person‑centered goals of care conversations that align treatment with what matters most to patients. Participants will learn communication skills to support shared decision‑making, trust, and high‑quality care across the illness trajectory.

Course Objectives:  Upon completion of the activity, the learner should be able to:

•Describe the purpose and value of person‑centered goals of care conversations in aligning medical treatment with patients’ values, goals, and quality‑of‑life priorities
•Apply evidence‑based communication frameworks (e.g., Ask‑Tell‑Ask, responding to emotion) to support shared decision‑making with patients and families.
•Identify appropriate timing, triggers, and documentation practices for goals of care conversations across care settings.
